The subthalamic nucleus is a small collection of neurons situated ventral to the thalamus i. It is a major component of the subthalamus. The subthalamic nucleus is considered part of the basal ganglia. The basal ganglia are a group of subcortical nuclei that are involved in a variety of cognitive and emotional functions, but are best known for their role in movement. To read more about the basal ganglia and this purported function, see this article.

Because the complex functions of the basal ganglia have been increasingly studied over the past several decades, the understanding of the role of the subthalamic nucleus STN in motor and cognitive functions has evolved. The increased attention focused on such non-motor functions housed within the STN partially arose from the observed cognitive and affective side effects seen with STN deep brain stimulation. The multiple modalities of research have corroborated these findings and have provided converging evidence that the STN is critically involved in cognitive processes. In particular, numerous experiments have demonstrated the involvement of the STN in high-conflict decisions. The different STN functions appear to be related to activity in anatomically distinct subregions, with the ventral STN contributing to high-conflict decision-making through its role in the hyperdirect pathway involving the prefrontal cortex. Here, we will review the literature that has led to this understanding of the role of the STN in cognition. We will begin by reviewing the anatomy of the STN and will describe various anatomic and computational models of the pathways of STN activity. We will discuss the results from animal experiments investigating the effect of STN lesions and stimulation on cognitive tasks. Then, we will review the clinical effects observed after STN DBS in terms of both cognitive and affective side effects. Next, we will turn to human experimental studies.
Most fibers connected bilaterally to Crus I in the cerebellum, while the remaining fibers crossed the midline and connected to Crus II. Hemiballismus is a relatively rare condition occurring as the sole manifestation of stroke in only roughly 0. It helps to modulate movement through the subthalamic nucleus pathway of the basal ganglia with the other components of the basal ganglia, subthalamic nucleus.
The subthalamic nuclei are small paired structures that are part of the functional basal ganglia. They are located ventral to the thalamus , dorsal to the substantia nigra and medial to the internal capsule. The subthalamic nucleus receives its main input from the lateral pallidum external segment of the globus pallidus. Excitatory, glutamatergic inputs come from the cerebral cortex particularly the motor cortex , and from the pars parafascicularis of the central complex. There are also neuromodulatory inputs from the substantia nigra pars compacta.

Several recent studies have focused on the autonomous pacemaking ability of subthalamic neurons. The purpose of this review is to synthesize the literature to date on the nonmotor functions of the STN. StatPearls [Internet]. Brain Research. Here, we will review the literature that has led to this understanding of the role of the STN in cognition. Based on this model of direct and indirect pathways and the role of dopamine depletion in the pathophysiology of PD, new insights were gained through animal models of PD into the role of the basal ganglia in the pathophysiology of movement disorders Figure 1B , middle. Hold your horses: a dynamic computational role for the subthalamic nucleus in decision making.
The subthalamic nucleus refers to an important aggregation of neuronal cell bodies, which is situated within the subthalamic region.
Stimulation of the subthalamic nucleus and impulsivity: release your horses. Kelly, R. However, it has been shown in thalamic slices from mice, [21] that the stimulus also causes nearby astrocytes to release adenosine triphosphate ATP , a precursor to adenosine through a catabolic process. Figure 3 Figure 3. Although these clinical trials mainly assess the impact of STN DBS on the entire population, the discrepancies with regard to effects of STN DBS on cognition and mood point to the difficulty in drawing conclusions regarding the actual cognitive or affective functions of the STN based on clinical effects, even in well-controlled studies. Paris: Bailliere. Although there are relatively few studies investigating the cognitive functions of the STN in nonhuman primates, STN neurons were demonstrated to increase the activity in response to visual cues indicating upcoming reward in a visually guided reward task Matsumura et al. The basal ganglia communicate with the cerebellum. Parafascicular nucleus projection to the extrastriatal basal ganglia in monkeys. In particular, the authors focused on the subthalamic region, with particular regard to STh connectivity. Despite evidencing largely closed loops within each functional domain, the authors discovered an unidirectional influence of the limbic over the motor loop via ventral striatum-substantia nigra SNr -motor thalamus circuitry; furthermore, activity within the ventral striatum of the rat seems to modulate the activity of the primary motor cortex.
