Lava can range in temperature from degrees Celsius 1, degrees Fahrenheit for some varieties to 1, degrees Celsius 2, degrees Fahrenheit or higher for others. As a result, it can be challenging to provide lava in DnD 5e with a precise temperature that precisely mimics the variation of real-world lava. My first inclination was to make the rules regarding lava as realistic as possible but eventually gave up. Here is why. You can try to make interactions as realistic as possible or you can give it more of a fantasy feel. As an example, here are two different ways I might come up with house rules for falling into lava. No saves.

Dexterity and Athletics: Characters with high Dexterity or Athletics scores may be able to make acrobatic or athletic maneuvers to avoid falling into lava or to quickly move through hazardous areas. The ability to think imaginatively and use the environment's perils and hazards is ultimately the key to doing harm to targets in a lava stream. Fire-immune creatures pass the saving throw. Perhaps the crafting of a new spell would be the best route. By doing so, they can lessen the harm they sustain by avoiding coming into contact with the lava. Originally Posted by Nifft. Re: More granular lava damage rules From a simulationist perspective, if you're allowing low-to-mid level people without magical protection to touch lava and live, something's probably gone wrong--lava is very, very hot, and unlike other things that are that hot like kilns and blast furnaces , there's a lot of hot stuff exposed to the air, heating up a big area around the lava. Of course, lava is quite dense and viscous, so even if you fall into it you probably take damage as though you are wading through it instead of being submerged for at least 10 rounds, methinks. Note: the damage is less than indicated in the DMG but I have added the no death saves and destroying all objects rules. Re: More granular lava damage rules. Additionally, if you are in a lava stream, you can use an action to make a Strength check to try and climb out or swim away from it.
