One aspect that has changed significantly is the concept of surprise. The first step of surprise is for the DM to determine who, if anyone, is surprised. Was the party trying to be stealthy i. If the party did not attempt to conceal their presence, then surprise is not an option. It should be noted that the party does not have to be aware of a particular group of enemies to attempt a stealth check. They can simply say they are being stealthy because their surroundings are inherently dangerous. For example, as the party descends into a dungeon, they can declare that they are proceeding with care because there is likely danger ahead. These Dexterity Stealth checks are compared to the passive Wisdom Perception scores of enemies within the dungeon. It is often a good idea for the DM to write down the passive perceptions of enemies to cut down on the time required to determine the outcome of the rolls. The wording surrounding stealth in 5e is vague enough to allow for either individual checks or a group check. A group check succeeds if at least half of the PCs succeed on their individual rolls.
Being Dnd surprised Combatants often find themselves lying on the ground, either because they are knocked down or because they throw themselves down.

Any character or monster that doesn't notice a threat is surprised at the start of the encounter.

No matter how many times I explain it to my friends, they still ask how it works the next time surprise is used in-game. Why is it so difficult to understand? I think surprise has been misinterpreted for several reasons: the PHB does a poor job explaining it, misinformation is confusing players, and the rules have room for interpretation. By the rules, 5e Surprise is based on an attacker being unseen and a target being unsuspecting. Being unseen allows an attacker to attack with advantage.

Whether it be positioning or setting up weapon combos, players have to engage in some semblance of planning. No plan is more common or more effective than trying to get the drop on an enemy. After all, if the enemy can't see the party coming, they'll have no time to prepare a counter attack. Instead, it is replaced with a new version of Surprised that, while helpful, is also exceedingly strong in comparison.

When does surprise actually come into play? It moves as you direct it, and it has only three action options: Dash , Disengage , and Dodge. Then add any relevant modifiers as normal. If neither side tries to be stealthy, they automatically notice each other. For example, if you have a speed of 30 and a flying speed of 60 because a wizard cast the fly spell on you, you could fly 20 feet, then walk 10 feet, and then leap into the air to fly 30 feet more. Combat begins when a character makes a hostile action. Any increase or decrease to your speed changes this additional movement by the same amount. Hit Points Hit points represent a combination of physical and mental durability, the will to live, and luck. Otherwise, you fail. Chapter nine of the PHB outlines combat rules and further explains rules for surprise. I had confused Alert and Observant there, I admit. The AC of a character is determined at character creation, whereas the AC of a monster is in its stat block. Most of this is completely straight forward except for one part. The first thing that jumps out is that one side needs to be actively trying to be stealthy. If the ranger has 14 current hit points and has a hit point maximum of 20, the ranger regains 6 hit points from the druid, not 8. The setting is urban, day and in a milling crowd on a a city street. I go with an active playing facing roll against a target DC for both occasions: when the party is being surprised perception and when the party is ambushing stealth. The increase equals your speed , after applying any modifiers.
Yes, really. I join told all above.
