Your GP will work out a care plan that is best suited to your needs in regards to managing your health needs. As Centre Health Complex already has EPC allied practitioners on site, implementing your new care plan is as easy as asking the receptionist to book your initial appointment for you. For your first visit with your allied health practitioner you will need to bring the EPC form which has been signed and given to you by your GP and the referral from your GP. For any consecutive visits your information will be kept on file so you will not be required to bring your referral each time.
